Financial Aid Form	2011-12	2012-13
Petition for Reinstatement of Assistance Use this form if you have been denied financial aid and you want to appeal that decision. Please note: you cannot appeal a previously denied appeal decision.
Award Adjustment Request Form Use this form to request cancellation of all aid, reduce or increase your loan amount and/or request a specific federal aid program.
Authorization Form Use this form to indicate your preference of how your federal funds are to be used.
Independent Verification Form This form is required for independent students that have been selected for verification. Use this form if you are 24 years old, married and/or have dependents.
Dependent Verification Form This form is required for dependent students that have been selected for verification. Use this form if you are under the age of 24, single with no dependents.
Direct Deposit Form Use this form to sign up for direct deposit of Financial Aid funds.
Consortium Agreement Request Form Use this form when applying for aid for combined enrollment at OSU-OKC & schools with whom we have consortium agreements.
Loan Reinstatement Request Form Use this form to request a reinstatement of your loan.
Request for Dependency Override Use this form to request dependency override.
Special Circumstance Form Use this form to report changes in your financial situation.
Parent Loan Authorization to Release Funds This form is to be completed by parents with PLUS loans to authorize the credit balance to be issued to the student.
Parental Resource Verification Form Use this form to provide parent & student income resources.
Student Resource Verification Form Use this form to provide student income resources.
Attendance Inquiry Form Use this form to inform the OSU-OKC Financial Aid and Scholarship Office if you plan to attend a specific semester.
Family Size Verification Form Use this form to document your family size.
Student Military Housing Allowance Form Use this form to report military housing allowance.
