Official OSU-OKC Name Badges
The new OSU-OKC name badges offer several options. You will choose from orange, white or silver with two or three lines of text. You will also have the option of a magnetic or a pin backing. Please consult with your supervisor before ordering if there’s a specific color or number of lines you must choose.
Follow these steps to expedite the ordering process.
- Complete the *Pre-approval Authorization Form provided by Purchasing to permit the use of your department’s P-card. *Vendor Name is Midwest Trophy.
- Download and fill out the Name Badge Order Form.
- Return the signed Pre-approval and ordering forms to Jamie Davis, Communications Department, Rm. 201C or email to jamie.davis@osuokc.edu.
Once the forms are received the order will be placed and your department’s account will be charged. The completed name badges will be shipped to Jamie Davis for approval prior to disbursement. If approved, your name badges along with original forms will be sent to your department via campus mail. You will need to keep these forms to reconcile your P-card accounts.
